Incident
Reporting

The Oregon TRIO Association (OTA) is committed to providing a safe, respectful, and inclusive environment for all. Whether you’re a board member, staff, student, volunteer, or guest at one of our events, your experience matters—and your voice deserves to be heard.

This page outlines our Incident Reporting Policy and gives you a direct way to report any concerns or misconduct.

What Happens Next

Step 1: Initial Review

OTA leadership will begin an assessment within five (5) business days of receiving your report.

Step 2: Investigation

If needed, an Incident Review Taskforce (IRT) will be formed to conduct a thorough investigation. The IRT may include impartial board members and, when applicable, a member of our DEI Taskforce. The investigation will typically conclude within 30 calendar days. If more time is needed, we’ll let you know.

Step 3: Resolution

Outcomes may include:

  • Formal warning or mediation
  • Suspension or removal from OTA leadership or membership
  • Restrictions from OTA events
  • Legal action if necessary

You will be notified of the outcome, within confidentiality guidelines.

Confidentiality & Retaliation

All reports and investigations are handled with strict confidentiality. Only those directly involved in the process will be informed. Retaliation is strictly prohibited. Anyone who retaliates against a person who files a report in good faith will face disciplinary action.

Appeals

If you disagree with the resolution, you may submit a written appeal within 14 calendar days of the decision. The OTA Board will respond within 30 calendar days with a final ruling.

OTA will securely store all incident reports and outcomes for a minimum of five (5) years.
